If you do not know the address and telephone number of the inviting person, please indicate the address and telephone number of the inviting company.

Item 33. Important point. You must specify who is covering the costs.

If you pay for yourself, then fill in the left part. Check the boxes for the available options.

If a sponsor (relative, employer or inviting company) pays for you, then fill in the right part.
