Application campaign for free trips for the holidays of Moscow children of preferential categories will take place in two stages. At the first stage, from November 2 to December 10, parents will choose destinations and rest periods.
After the application passes to the second stage, parents will be able to book a trip to a specific base and choose the date of arrival. The second stage of the application campaign will last from February 7 to February 21, 2018.

“The two-step booking system was introduced in 2017 and has proven to be very effective. Parents submitted applications in a relaxed manner, which reduced the number of errors when filling out. This year, at the request of parents, for the 2018 summer health campaign, we have increased the time to submit an application from two weeks to five. This will make it possible to conduct competitive procedures and select recreation centers based on the needs of Moscow children. As in 2017, for a number of benefit categories (except for orphans and children with disabilities), a choice of monetary compensation in the amount of 30,000 rubles is available. After a positive decision on the application is made, the voucher for the recreation and health of children will be sent to the applicant by March 15, 2018, and the money for the certificate will be transferred by March 18, 2018.

1. General Provisions
1.1. Maintaining the Unified City Register of citizens who have the right to be provided with medicines and medical products free of charge or with a 50% discount in the city of Moscow (hereinafter referred to as the LO Register) is carried out in order to streamline the provision of medicines and medical products, as well as improve the quality of service for citizens who have in accordance with current legislation, the right to receive state social assistance in the city of Moscow.
1.2. The Leningrad Region Register is a state information resource of the city of Moscow.
1.3. The LO Register includes:
1.3.1. Citizens entitled to receive state social assistance in the form of a set of social services in accordance with Federal Law dated July 17, 1999 N 178-FZ “On State Social Assistance” in terms of providing necessary medications with prescriptions for drugs, medical products with prescriptions for medical products in accordance with standards medical care, as well as specialized medical nutrition products for disabled children and included in the regional segment of the Federal Register.
1.3.2. 1.4. Inclusion of a citizen in the Register of Leningrad Regions is the basis for issuing electronic prescriptions for him to receive medicines and medical products free of charge or with a 50% discount using functionality EMIAS, as well as for registration in EMIAS of prescriptions written by doctors in the name of a citizen using printed forms.
1.5. This Procedure does not apply to:
1.5.1. 1.5.2. To maintain the Register of LPs in terms of recording citizens suffering from life-threatening and chronic progressive rare (orphan) diseases leading to a reduction in the life expectancy of citizens or their disability (Resolution of the Government of the Russian Federation dated April 26, 2012 N 403 “On the procedure for maintaining the federal register of persons suffering from life-threatening diseases” and chronic progressive rare (orphan) diseases leading to a reduction in the life expectancy of citizens or their disability, and its regional segment."
2. Information support for maintaining the LP register
2.1. The Medical Register is maintained using the functionality of the Unified Medical Information and Analytical System of the City of Moscow (hereinafter referred to as EMIAS).
2.2. Entering information about citizens into the Register of Leningrad Regions is carried out by medical organizations state system Healthcare of the city of Moscow, to which citizens are assigned to receive primary health care as part of the implementation of the choice by the insured person medical institution, implementing the territorial program of compulsory health insurance(OMS), in accordance with the legislation of the Russian Federation and the city of Moscow and stationary state social service institutions of the Department social protection population of the city of Moscow. The list of medical organizations authorized to maintain the Register of Medical Institutions is approved by the Moscow Department of Health.
2.3. Entering information about citizens regarding certain categories of diseases, for the outpatient treatment of which drugs and medical products are dispensed free of charge according to a doctor's prescription, is carried out by medical organizations providing primary specialized health care for the corresponding profile of diseases. The list of medical organizations authorized to maintain the Register of Medical Infections for certain categories of diseases is approved by the Moscow Department of Health.
2.4. If a citizen is constantly observed in a medical organization that is not included in the list of authorized medical organizations for maintaining the Register of Medical Institutions, information about him is entered into the Register of Medical Institutions by the authorized medical organization at the place of residence or, at the request of the citizen, by the authorized medical organization of his choice after being attached to this organization in the prescribed manner.
2.5. Control over the maintenance of the Register of Medicines and organizational and methodological support of the processes of maintaining the register is carried out by the Information and Analytical Service of the State Budgetary Healthcare Institution "Center for Drug Supply and Quality Control of the Moscow City Health Department" (hereinafter referred to as the IAS GBUZ "TsLO and QC DZM").
2.6. Technical and consulting support for the processes of maintaining the Register of Medical Entities is provided by the EMIAS Support Service.
2.7. Persons authorized to maintain the Register of medicinal products in a medical organization are appointed by orders of the head of the medical organization. Authorized persons bear full responsibility for the accuracy of the information they entered into the Register of Medical Institutions. Copies of orders for the appointment of authorized persons, as well as changes in the list of authorized persons, are sent to the Information Information System of the State Budgetary Institution of Healthcare "TsLO and KK DZM" by fax: 8-495-974-92-79 or in scanned form by e-mail: [email protected].
2.8. The procedure for receiving citizens to enter information into the Register of Medical Institutions (at the registry or by a dedicated specialist) is established by the head of the relevant medical organization and is brought to the attention of medical workers receiving patients and citizens (by posting the relevant information on information stands in the medical organization).
3. Transitional provisions
3.1. The transition to maintaining the Medical Register using EMIAS is carried out in stages in groups of medical organizations in accordance with the schedule for implementing L O EMIAS services.
3.2. Before the transition to maintaining the Register of Medical Products using EMIAS, medical organizations submit to the Information Information System of the State Budgetary Healthcare Institution "TsLO and KK DZM" by fax or e-mail a list of persons authorized to maintain the Register of Medical Products with contact information: last name, first name, patronymic, position, work and mobile phones, E-mail address.
3.3. The initial filling of the LO EMIAS Register is carried out on the basis of information maintained by LLC "Gmktsrit" on the basis of the order of the Moscow City Health Committee dated 02.12.2001 N 65 "On the preparation of the Unified City Register of the population entitled to preferential and free dispensing of medicines and medical products ". Loading of information from Gmktsrit LLC is carried out by the Information Information System of the State Budgetary Institution of Healthcare "TsLO and KK DZM" in accordance with the approved schedule for the implementation of EMIAS services. Based on the results of downloading information and reconciling it with the Unified Register of Insured Persons of the Moscow City Compulsory Health Insurance Fund (hereinafter referred to as the ERZ), the IAS GBUZ "CLO and QC DZM" generates download reports for each medical organization with a list of identified inconsistencies and unidentified citizens.
3.4. Within no more than 10 working days from the date of receipt of the loading report, the medical organization will organize the reconciliation of the data on the reports using available paper documentation and the addition of data on the citizens indicated in the report to the Register of Leningrad Regions.
3.5. Based on the results of the reconciliation, a report is drawn up on the readiness of the medical organization to maintain the Register of Medical Institutions using EMIAS.
3.6. From the moment of downloading data from LLC "Gmktsrit" until the moment of drawing up the Act, the medical organization ensures the parallel entry of new information about citizens both into EMIAS and using the software of LLC "Gmktsrit". Entering information into Gmktsrit LLC ceases from the moment the Act is drawn up.
3.7. During the transition period, it is allowed to enter data on federal categories of citizens into the Register of Leningrad Regions without submitting certificates from the Pension Fund of Russia, based on information from LLC Gmktsrit (by registering in the Register of Leningrad Region the electronic document “Information on reconciliation with LLC Gmktsrit”).
4. General rules for maintaining the Register
4.1. Entry (change) of data on a citizen’s benefits into the Register of LR is carried out upon his personal appeal to the medical organization authorized to maintain the Register of LR, to which the citizen is attached.
4.2. The Register of Leningrad Regions includes citizens of the Russian Federation who permanently reside in the city of Moscow and are included in the ERZ. In the absence of a citizen in the ERZ, before making an entry in the Register of Leningrad Regions, the citizen is registered in the ERZ.
4.3. The Register of Leningrad Regions does not process personal data of citizens. Records of the grounds for receipt are stored in the LO Register in an anonymized form. Identification of citizens in the Register of Leningrad Regions is carried out according to the ERZ data on the basis of their consent to the processing of personal data received upon registration in the ERZ.
4.4. Before entering information into the Register of Medical Institutions, a citizen must be attached to a medical organization in accordance with the order of the Ministry of Health and Social Development of Russia dated April 26, 2012 N 406n “On approval of the Procedure for a citizen to select a medical organization when providing him with medical care within the framework of the program of state guarantees of free provision of medical care to citizens.” A citizen is assigned to an outpatient center - an association of several medical organizations that are part of one legal entity.
4.5. Attachment of citizens in certain categories of diseases is carried out for observation by narrow specialists in medical organizations that are not part of the outpatient center by making an appropriate entry (electronic certificate of attachment) in the Register of LOs based on the diagnosis established in the authorized medical organization.
4.6. A citizen may have the right to receive drug coverage in one or more federal and regional categories that do not contradict each other. All categories to which a citizen can be classified are entered into the LO Register; control of the consistency of categories is ensured automatically by EMIAS software.
4.7. Entry of data into the Register of Leningrad Regions is carried out on the basis of supporting documents submitted by citizens, or on the basis electronic documents, coming from other government information resources. Details of supporting documents submitted by citizens are entered into the Register of LR by authorized persons of medical organizations. Determining the composition and validity period of the categories to which a citizen can be classified based on the submitted documents is performed automatically by EMIAS software.
4.8. All documents are entered into the LO Register once. If a citizen has grounds for assigning a new category, only supporting documents that are not in the Register of Leningrad Regions are subject to registration in EMIAS.
4.9. Expired documents are not deleted from the LO Register.
4.10. Full list categories of citizens entitled to receive medicines and medical products free of charge or with a 50 percent discount in the city of Moscow, indicating the grounds for entering data about them into the Register of Medical Products (types of supporting documents) is given in this Procedure.
5. Features of recording data on federal categories of citizens
5.1. Data on federal categories of citizens are entered into the Regional Register on the basis of information from the regional segment of the Federal Register of Persons entitled to receive state social assistance (hereinafter referred to as the Federal Register). Medical workers (doctors and nurses during a patient’s appointment) and authorized persons of a medical organization (during the entry of information into the Register of Medical Institutions), following oral requests from citizens, can request UMIAS to update data from the Federal Register. Information about a citizen’s belonging to federal categories is entered into the Register of Leningrad Regions automatically based on the results of the request.
5.2. Requesting information about a citizen from the Federal Register is allowed no more than once a day; the number of requests is automatically limited by EMIAS.
5.3. If there is no information in the regional segment of the Federal Register about a citizen’s right to receive medicines and medical products free of charge or with a 50% discount, the information can be entered into the Regional Register by an authorized person of a medical organization on the basis of a certificate submitted by the citizen from the territorial office of the Pension Fund of Russia on receipt monthly cash payment in the form approved by Resolution of the Board of the Pension Fund of November 2, 2006 N 261p “On organizing the work of the Pension Fund of the Russian Federation and its territorial bodies to provide citizens with documents confirming their right to receive a set of social services (social services)” (hereinafter referred to as a certificate of receipt of EDV ). The validity period of the category is set in accordance with the validity period of the certificate.
5.4. The grounds for termination of a citizen’s federal category are:
5.4.1. Receipt from the Federal Register of information about the termination of the category.
5.4.2. Submission by the citizen of a certificate of renunciation of EDV.
5.4.3. Expiration of the validity period of the certificate of receipt of EDV in the absence of information about the presence of the category from the Federal Register.
5.4.4. Submission by a citizen of a certificate of receipt of EDV in a category that contradicts the previously registered one.
6. Features of recording data on regional categories of citizens suffering from diseases for which outpatient treatment drugs and medical products are dispensed according to a doctor’s prescription free of charge or with a 50% discount
6.1. Entering into the Register of Leningrad Regions information about citizens suffering from diseases, for the outpatient treatment of which drugs and medical products are dispensed according to a doctor’s prescription free of charge or with a 50% discount, is carried out on the basis of a decision of the medical commission of the medical organization to which the citizen is attached to provide primary healthcare. sanitary care, documented in a protocol (extract from the protocol).
6.2. For certain categories of diseases specified in the “Notes” column to this Procedure, information may be entered by authorized specialized medical organizations, the list of which is approved by the Moscow Department of Health.
6.3. The protocol of the decision of the medical commission must indicate:
6.3.1. Code of diagnosis that gives a citizen the right to be provided with medicines and medical products free of charge or with a 50% discount, according to the International Classification of Diseases, 10th revision (ICD-10) in accordance with this Procedure.
6.3.2. Last name, first name, patronymic, position, signature and personal seal of the doctor - chairman of the medical commission that established the diagnosis.
6.3.3. Date of re-examination (validity period of the category), the indication “indefinitely” is allowed.
6.4. If the diagnosis is established by a medical organization that is not one of those authorized to maintain the Medical Medical Register, medical documentation confirming the established diagnosis is submitted by the citizen or his representative to the medical commission of the authorized medical organization. To confirm the diagnosis, the medical commission, if necessary, may request additional medical documents and refer the citizen for additional examination within the framework of the territorial compulsory health insurance program.
6.5. The grounds for termination of registration due to illness are:
6.5.1. Expiration of the category if the medical commission set a period for re-examination to confirm the diagnosis, and the citizen did not appear for re-examination.
6.5.2. Conclusion of a medical commission on cure or change in diagnosis.
7. Termination of accounting
7.1. The grounds for terminating the validity of the information from the Register of Leningrad Regions about a citizen’s membership in the number of persons entitled to receive medicines and medical products free of charge or with a 50% discount are:
7.1.1. Expiration of the established period of validity of the right to receive medicines and medical devices free of charge or with a 50% discount (including the age limit up to which the benefit is valid).
7.1.2. Termination of the circumstances on the basis of which the citizen is assigned to the appropriate category (healing, completion of training, and so on).
7.1.3. Termination permanent residence in the city of Moscow (departure from the number of residents of the city of Moscow).
7.1.4. A written refusal of a citizen to exercise the right to receive medicines and medical products free of charge or with a 50% discount.
7.2. Termination of validity of information about a citizen in the Register of Leningrad Regions is carried out:
7.2.1. Automatically based on data entered into the Register of Leningrad Regions, received by EMIAS from the ERZ or from the Federal Register.
7.2.2. Based on documented information entered into the Register of Medical Institutions by an authorized person of a medical organization or an authorized specialist of the Information Information System of the State Budgetary Institution of Healthcare "CLO and QC DZM" as part of control over the maintenance of the Register of Medical Institutions.
7.3. The grounds for termination of registration of a citizen in the Register of Leningrad Regions are:
7.3.1. Death of the registered person.
7.3.2. Identification of erroneous records (records for non-existent persons or persons not identified in the ERZ).
7.3.3. Identification of duplicate records about the same person (in relation to the duplicate record with the simultaneous exclusion of the duplicate from the individual legal record).
7.3.4. Application of a citizen who is the subject of an entry in the Register of Leningrad Regions.
7.4. To ensure retrospective accounting, records for which accounting has been discontinued are not deleted, but are transferred to the archive. Translation of records into the archive is carried out by authorized persons of medical organizations using EMIAS services.
7.5. It is not permitted to transfer records to the archive due to the expiration of the benefit, the citizen’s departure from the city of Moscow, separation from a medical organization, or other grounds that suggest the possibility of renewing the right to receive medicines and medical devices in the future. If a citizen loses the right to receive medicines and medical products free of charge or with a 50% discount, the authorized person registers the termination of the category in the Register of LR.
7.6. Removal of the “archive” status from an entry in the Register of Medical Institutions if it is erroneously assigned is carried out by specialists of the Information Information System of the State Budgetary Healthcare Institution “TsLO and KK DZM” on the basis of written requests from authorized persons of medical organizations.
7.7. The destruction of archival records of the LO Register is carried out in accordance with general rules archival storage of medical documentation.

The unified city register of citizens entitled to be provided with medicines and medical products free of charge or with a 50% discount is maintained with the aim of streamlining the provision of medicines and medical products, improving the quality of service for citizens entitled to receive state social assistance.
The inclusion of a citizen in the register is the basis for issuing electronic prescriptions for him to receive medicines and medical products for free or with a 50 percent discount using the functionality of the Unified Medical Information and Analytical System of the city, as well as for registering in it prescriptions written by doctors in the name of the citizen using printed forms.
Information about citizens is entered into the register by medical organizations to which citizens are attached, or by medical organizations providing primary specialized health care for the relevant profile of diseases. The list of organizations authorized to maintain the Register is approved by the Department of Health. If a citizen is observed in a medical organization that is not included in the list, information about him is entered by the authorized organization at the place of residence or, at the request of the citizen, by an authorized organization of his choice after joining this organization.
