1. General Provisions

1.1. The maintenance of the Unified City Register of Citizens Entitled to Provision of Medicines and Medical Devices Free of Charge or at a 50% Discount in the City of Moscow (hereinafter referred to as the LO Register) is carried out in order to streamline the provision medicines and medical devices, as well as improving the quality of service for citizens who, in accordance with applicable law, are entitled to receive state social assistance in the city of Moscow.

1.2. Register LO is the state information resource of the city of Moscow.

1.3. The LO Register includes:

1.3.1. Citizens entitled to receive state social assistance in the form of a set of social services in accordance with the Federal Law of July 17, 1999 N 178-FZ "On State Social Assistance" in terms of providing the necessary medicines under prescriptions for medicines, medical products under prescriptions for medical devices, in accordance with the standards medical care, as well as specialized medical nutrition products for children with disabilities and included in the regional segment of the Federal Register.

1.3.2. Residents of the city of Moscow who are entitled to social support measures in the form of providing medicines and medical devices free of charge or with a 50% discount on doctor's prescriptions in accordance with the Law of the city of Moscow dated 03.11.2004 N 70 "On measures of social support for certain categories of residents of the city of Moscow "and the order of the Government of Moscow dated 10.08.2005 N 1506-RP "On the implementation of social support measures for certain categories of residents of the city of Moscow to ensure medicines and medical products dispensed on prescription free of charge or at a 50 percent discount.

1.4. The inclusion of a citizen in the Register of LP is the basis for issuing electronic prescriptions for him to receive medicines and medical devices free of charge or at a 50% discount using functionality EMIAS, as well as for registration in EMIAS of prescriptions issued by doctors in the name of a citizen using printed forms.

1.5. This Order does not apply:

1.5.1. To maintain the Register of LP in terms of accounting for persons with hemophilia, cystic fibrosis, pituitary dwarfism, Gaucher's disease, malignant neoplasms of lymphoid, hematopoietic and related tissues, multiple sclerosis, persons after organ and (or) tissue transplantation, the organization of the provision of which with drugs is carried out according to list approved by the Government Russian Federation and formed in the manner prescribed by it (Decree of the Government of the Russian Federation of April 26, 2012 N 404 "On approval of the Rules for maintaining the Federal Register of Persons with Hemophilia, Cystic Fibrosis, Pituitary Dwarfism, Gaucher's Disease, Malignant Neoplasms of the Lymphoid, Hematopoietic and Related Tissues, Multiple Sclerosis, persons after transplantation of organs and (or) tissues).

1.5.2. To maintain the Register of LP in terms of recording citizens suffering from life-threatening and chronic progressive rare (orphan) diseases that lead to a reduction in life expectancy of citizens or their disability (Decree of the Government of the Russian Federation dated April 26, 2012 N 403 "On the procedure for maintaining the federal register of persons suffering from life-threatening and chronic progressive rare (orphan) diseases, leading to a reduction in life expectancy of citizens or their disability, and its regional segment").

2. Information support for maintaining the register of legal entities

2.1. The LO register is maintained using the functionality of the Unified Medical Information and Analytical System of the City of Moscow (hereinafter - EMIAS).

2.2. Entering information about citizens into the Register of LO is carried out by medical organizations state system health care of the city of Moscow, to which citizens are attached to receive primary health care as part of the implementation of the choice by the insured person medical institution, which implements the territorial program of mandatory health insurance(OMS), in accordance with the legislation of the Russian Federation and the city of Moscow and stationary state social service institutions of the Department of Social Protection of the Population of the City of Moscow. The list of medical organizations authorized to maintain the LR Register is approved by the Moscow City Health Department.

2.3. Entering information about citizens in terms of certain categories of diseases, in the outpatient treatment of which drugs and medical devices are dispensed free of charge by prescription, is carried out by medical organizations providing primary specialized health care for the corresponding profile of diseases. The list of medical organizations authorized to maintain the LR Register in terms of certain categories of diseases is approved by the Moscow City Health Department.

2.4. If a citizen is constantly observed in a medical organization that is not included in the list of authorized medical organizations for maintaining the LP Register, information about him is entered into the LP Register by an authorized medical organization at the place of residence or, at the request of a citizen, by an authorized medical organization of his choice after being attached to this organization in the prescribed manner.

2.5. Control over the maintenance of the Register of LP and organizational and methodological support for the processes of maintaining the register is carried out by the Information and Analytical Service of the State Budgetary Health Institution "Center for Drug Supply and Quality Control of the Moscow City Health Department" (hereinafter - IAS GBUZ "TsLO and KK DZM").

2.6. Technical and consulting support for the processes of maintaining the LP Register is carried out by the EMIAS Support Service.

2.7. Persons authorized to maintain the LP Register in a medical organization are appointed by orders of the head of the medical organization. Authorized persons bear full responsibility for the accuracy of the information entered by them in the LR Register. Copies of orders on the appointment of authorized persons, as well as changes in the list of authorized persons, are sent to the IAS GBUZ "TsLO and KK DZM" by fax: 8-495-974-92-79 or in scanned form by e-mail: [email protected]

2.8. The procedure for the admission of citizens for entering information into the Register of LP (in the registry or by a dedicated specialist) is established by the head of the relevant medical organization and brought to the attention of medical workers who receive patients and citizens (by posting relevant information on information boards in a medical organization).

3. Transitional provisions

3.1. The transition to maintaining the LP Register using UMIAS is carried out in stages in groups of medical organizations in accordance with the schedule for the implementation of LO UMIAS services.

3.2. Prior to the start of the transition to maintaining the LP Register using EMIAS, medical organizations submit to the IAS GBUZ "TsLO and KK DZM" by fax or e-mail a list of persons authorized to maintain the LP Register with contact details: last name, first name, patronymic, position, worker and mobile phones, E-mail address.

3.3. The initial filling of the Register of LO EMIAS is carried out on the basis of information maintained by Gmktsrit LLC on the basis of the order of the Moscow City Health Committee dated February 12, 2001 N 65 "On the preparation of the Unified City Register of the population entitled to preferential and free distribution of medicines and medical products ". Downloading information from Gmktsrit LLC is carried out by the IAS GBUZ TsLO and KK DZM in accordance with the approved schedule for the implementation of EMIAS services. Based on the results of uploading information and reconciling it with the Unified Register of the Insured of the Moscow City Compulsory Medical Insurance Fund (hereinafter - ERZ), the IAS GBUZ "TsLO and KK DZM" generates loading reports for each medical organization with a list of identified inconsistencies and unidentified citizens.

3.4. Within a period of no more than 10 working days from the date of receipt of the loading report, the medical organization organizes the reconciliation of the data on the reports using the available paper documentation and the submission of data on the citizens indicated in the report to the Register of LO.

3.5. Based on the results of the reconciliation, an Act is drawn up on the readiness of the medical organization to maintain the LO Register using EMIAS.

3.6. From the moment the data is loaded from Gmktsrit LLC until the moment the Act is drawn up, the medical organization ensures the parallel entry of new information about citizens both into EMIAS and using Gmktsrit LLC software. Entering information into LLC "Gmktsrit" is terminated from the moment the Act is drawn up.

3.7. During the transition period, it is allowed to enter data on federal categories of citizens into the LO Register without submitting certificates from the Pension Fund of Russia, on the basis of information from Gmktsrit LLC (by registering an electronic document "Information on reconciliation with Gmktsrit LLC" in the LO Register).

4. General rules for maintaining the Register

4.1. The entry (change) of data on the benefits of a citizen in the LP Register is carried out upon his personal application to a medical organization authorized to maintain the LP Register, to which this citizen is attached.

4.2. The LO Register includes citizens of the Russian Federation permanently residing in the city of Moscow and included in the ERZ. In the absence of a citizen in the ERZ, before making an entry in the LO Register, a citizen is registered in the URZ.

4.3. The LO Register does not process personal data of citizens. Records on the grounds for receipt are stored in the LO Register in an impersonal form. Identification of citizens in the LR Register is carried out according to the URZ data on the basis of their consent to the processing of personal data obtained during registration in the URZ.

4.4. Before entering information into the LO Register, a citizen must be attached to a medical organization in accordance with the order of the Ministry of Health and Social Development of Russia dated April 26, 2012 N 406n "On approval of the procedure for choosing a medical organization by a citizen when providing him with medical care under the program of state guarantees of free provision of medical care to citizens ". A citizen is attached to an outpatient center - an association of several medical organizations that are part of one legal entity.

4.5. Attachment of citizens in terms of certain categories of diseases is carried out for observation by narrow specialists in medical organizations that are not part of the outpatient center, by making an appropriate entry (electronic certificate of attachment) in the Register of LO based on the diagnosis established in an authorized medical organization.

4.6. A citizen may be entitled to receive drug coverage under one or more federal and regional categories that do not contradict each other. All categories to which a citizen can be assigned are entered into the LO Register, the control of the consistency of categories is provided automatically by the EMIAS software.

4.7. Entering data into the LP Register is carried out on the basis of supporting documents submitted by citizens, or on the basis of electronic documents coming from other state information resources. Details of supporting documents submitted by citizens are entered into the Register of LO by authorized persons of medical organizations. The determination of the composition and validity of the categories to which a citizen can be assigned on the basis of the submitted documents is performed automatically by EMIAS software.

4.8. All documents are entered into the LO Register once. If a citizen has grounds for assigning a new category, only supporting documents that are not in the LO Register are subject to registration in the EMIAS.

4.9. Documents that have become invalid are not deleted from the LR Register.

4.10. Full list categories of citizens entitled to receive medicines and medical devices free of charge or at a 50% discount in the city of Moscow, indicating the grounds for entering data about them in the Register of LO (types of supporting documents) is given in this Procedure.

5. Features of accounting for data on federal categories of citizens

5.1. Data on federal categories of citizens are entered into the Register of LOs on the basis of information from the regional segment of the Federal Register of Persons Eligible for State Social Assistance (hereinafter referred to as the Federal Register). Medical workers (doctors and nurses during the admission of the patient) and authorized persons of the medical organization (in the course of entering information into the Register of LO), upon oral requests from citizens, can request UMIAS to update data from the Federal Register. Information about the citizen's belonging to the federal categories is entered into the Register of the Leningrad Region based on the results of the request automatically.

5.2. A request for information about a citizen from the Federal Register is allowed no more than once a day, the number of requests is automatically limited by EMIAS.

5.3. If there is no information in the regional segment of the Federal Register about the right of a citizen to receive medicines and medical devices free of charge or at a 50% discount, the information can be entered into the LO Register by an authorized person of a medical organization on the basis of a certificate submitted by a citizen from the territorial office of the Pension Fund of Russia on receipt monthly cash payment in the form approved by the Resolution of the Board of the Pension Fund of the Russian Federation dated November 2, 2006 N 261p "On the organization of the work of the Pension Fund of the Russian Federation and its territorial bodies to provide citizens with documents confirming their right to receive a set of social services (social services)" (hereinafter referred to as the certificate of receipt ). The validity period of the category is set in accordance with the validity period of the certificate.

5.4. The grounds for terminating a federal category for a citizen are:

5.4.1. Receipt from the Federal Register of information on the termination of the category.

5.4.2. Submission by a citizen of a certificate of refusal from the EDV.

5.4.3. Expiration of the certificate of receipt of the EDV in the absence of information about the presence of a category from the Federal Register.

5.4.4. Submission by a citizen of a certificate of receipt of the EDV in a category that contradicts the previously registered one.

6. Features of recording data on regional categories of citizens suffering from diseases, in the outpatient treatment of which medicines and medical devices are dispensed by prescription free of charge or with a 50% discount

6.1. Information about citizens suffering from diseases, in the outpatient treatment of which medicines and medical devices are dispensed by prescription free of charge or at a 50% discount, is entered into the LR Register based on the decision of the medical commission of the medical organization to which the citizen is attached to provide primary medical care. sanitary care, drawn up by the protocol (extract from the protocol).

6.2. For certain categories of diseases specified in the "Notes" column to this Procedure, it is allowed to enter information by authorized specialized medical organizations, the list of which is approved by the Moscow City Health Department.

6.3. The protocol of the decision of the medical commission should indicate:

6.3.1. Diagnosis code that entitles a citizen to the provision of medicines and medical devices free of charge or at a 50% discount, according to the 10th revision of the International Classification of Diseases (ICD-10) in accordance with this Procedure.

6.3.2. Surname, name, patronymic, position, signature and personal seal of the doctor - chairman of the medical commission that established the diagnosis.

6.3.3. Date of re-examination (validity period of the category), it is allowed to indicate "indefinitely".

6.4. If the diagnosis is established by a medical organization that is not among those authorized to maintain the LO Register, medical documentation confirming the established diagnosis is submitted by a citizen or his representative to the medical commission of an authorized medical organization. To confirm the diagnosis, the medical commission, if necessary, may request additional medical documents and send the citizen for an additional examination under the territorial program of compulsory medical insurance.

6.5. The grounds for termination of registration for the disease are:

6.5.1. The expiration of the category, if the medical commission set a re-examination period to confirm the diagnosis, and the citizen did not appear for re-examination.

6.5.2. The conclusion of the medical commission on the cure or change in diagnosis.

7. Termination of accounting

7.1. The grounds for the termination of the information of the Register of LO on the belonging of a citizen to the number of persons entitled to receive medicines and medical devices free of charge or with a 50% discount are:

7.1.1. Expiration of the established period of validity of the right to receive medicines and medical devices free of charge or at a 50% discount (including the onset of the age limit until which the benefit is valid).

7.1.2. Termination of the circumstances on the basis of which a citizen is assigned to the appropriate category (cure, completion of training, and so on).

7.1.3. Termination permanent residence in the city of Moscow (withdrawal from among the residents of the city of Moscow).

7.1.4. Written refusal of a citizen to exercise the right to receive medicines and medical devices free of charge or with a 50% discount.

7.2. Termination of the validity of information about a citizen in the LO Register is carried out:

7.2.1. Automatically based on the data entered in the LO Register, received by EMIAS from the ERZ or from the Federal Register.

7.2.2. Based on documented information entered into the LP Register by an authorized person of a medical organization or an authorized specialist of the IAS GBUZ "TsLO and KK DZM" as part of control over the maintenance of the LP Register.

7.3. The grounds for terminating the registration of a citizen in the LO Register are:

7.3.1. Death of the registrant.

7.3.2. Identification of erroneous records (records for non-existent persons or persons not identified in the URZ).

7.3.3. Identification of duplicate entries about the same person (in relation to the duplicate entry with the simultaneous exclusion of the duplicate from the URZ).

7.3.4. Statement of a citizen who is the subject of an entry in the LO Register.

7.4. To ensure retrospective accounting, records for which accounting is stopped are not deleted, but transferred to the archive. The transfer of records to the archive is carried out by authorized persons of medical organizations using the UMIAS services.

7.5. It is not allowed to transfer records to the archive in connection with the expiration of the benefit, the departure of a citizen from the city of Moscow, detachment from a medical organization, or other reasons suggesting the possibility of renewing the right to receive medicines and medical devices in the future. If a citizen loses the right to receive medicines and medical devices free of charge or at a 50% discount, an authorized person registers the termination of the category in the LO Register.

7.6. The removal of the status "archived" from an entry in the Register of LO in case of its erroneous assignment is carried out by specialists of the IAS GBUZ "TsLO and KK DZM" on the basis of written requests from authorized persons of medical organizations.

7.7. Destruction of archival records of the LO Register is carried out in accordance with general rules archival storage of medical records.

The inclusion of a citizen in the register is the basis for issuing electronic prescriptions for receiving medicines and medical devices free of charge or at a 50% discount using the functionality of the Unified Medical Information and Analytical System of the city, as well as for registering prescriptions written by doctors in the name of a citizen in it using printed forms.

Information about citizens is entered into the register by medical organizations to which citizens are attached, or by medical organizations providing primary specialized health care for the relevant profile of diseases. The list of organizations authorized to maintain the Register is approved by the Department of Health. If a citizen is observed in a medical organization that is not included in the list, information about him is entered by an authorized organization at the place of residence or, at the request of a citizen, by an authorized organization of his choice after being attached to this organization.

MOSCOW, March 10. /TASS/. First stage bid campaign to receive vouchers paid from the budget of the city of Moscow for children of preferential categories begins in the capital. Two phases of the campaign

"This year, for the first time, the booking of vouchers for children's holidays, paid from the budget of the city of Moscow, will take place in two stages. At the first stage, within two weeks from 10 to 24 March, parents will be able to apply for a voucher, indicating the type of holiday, the number of children in the family and preferential category.Also, at the first stage of booking, parents need to select three regions to which they would like to send their child and determine three options for the time of rest.Muscovites can apply for a preferential voucher in the "services and services" section, the press service of the Moscow City Tour.
